Team Member of the Month
The DHSS Team Member of the Month program recognizes team members for outstanding service and/or a special achievement.
How to nominate: Complete and submit a Team Member of the Month Nomination Form to the Team Member of the Month coordinator. Each nomination is considered for 6 consecutive months.
Who can nominate: Any DHSS team member can nominate another DHSS team member. Department directors, deputy directors, division directors, deputy division directors (acting and official), and hourly and intermittent (H&I) team members are not eligible for the Team Member of the Month award.
Reward: The team member selected Team Member of the Month will be presented with a plaque at a reception hosted by the Department Director. These team members will also be considered for the State Team Member of the Month award.
